Medical Records

Outpatient Records ensures active duty service members and their dependents’ medical records are maintained and stored in the correct location.

Although the medical record contains private health information about the patient, the original record is the property of the United States government and must be maintained in the military treatment facility. Medical information is carefully safeguarded in accordance with applicable laws. Original medical records will not be released outside the Military Health System, or without your consent except in rare events.

Key Services Provided by Medical Records:

  • Mailing of medical records
  • In-processing hand carried medical records
  • Out-processing members for PCS, separation, or retirement
  • Releasing medical information as appropriate and/or requested
  • Uploading paper medical records to a patient’s electronic health record including off base medical records

For out-processing Medical records

Please bring a copy of your orders to TRICARE/Patient Administration Front Desk in Building 2114, Room 122a to be signed off vMPF.

How to Request Medical Records

To request a copy of medical records, visit Outpatient Records and complete a DD Form 2870.
