Medical Records
Outpatient Records ensures active-duty service members and their dependents’ medical records are maintained and stored in the correct location.
Ramstein Outpatient Records consists of three active-duty service members and one civilian contractors.
Key Services Provided by Medical Records:
- Mailing of medical records
- In-processing hand carried medical records
- Out-processing members for PCS, separation, or retirement
- Releasing medical information as appropriate and/or requested
- Uploading paper medical records to a patient’s electronic health record including off base medical records
How to Request Medical Records
To request a copy of medical records, visit Outpatient Records and complete a
DD Form 2870
